Student Locker Inventory Checklist

The following list is comprehensive of every item you should have in your assigned locker.

Students taking 1135,1142, 2150, 2130, or 3410 chemistry labs should fill this form out twice for each lab they are in: once at the start of the semester and once at the end of the semester. It is to ensure you have all the equipment you need, as well as to keep track of any lost or damaged items.

When you initially get your assigned locker, if you have broken or missing items, go to the stockroom to get replacements before submitting this form.

All forms submitted for the first time at the start of the semester must have all items accounted for. You are responsible for getting these items from the stockroom to have a complete inventory in your locker. You will not be charged for items that were not in your locker when you initially got your locker assignment.

At the end of the semester, if any items are lost or damaged that were assigned to you, you will be responsible for paying for those items.

BY SUBMITTING BELOW, I UNDERSTAND AND ACCEPT:

That I have thoroughly inspected my locker for both the correct quantity of items listed and that all items in the locker are in good condition. I will agree to and comply with all conditions listed in the Release and Indemnity and Student Acceptance of Financial Responsibility Agreements. I will be charged for any items broken or lost AFTER INITIAL CHECK IN, including damaged or missing items not noticed during initial inspection. Lost or broken items will only be replaced with department approved items through the stockroom. Items purchased from alternative sources is strictly prohibited. In addition to a $50 fee, any persons found replacing items purchased from an alternative source will be charged the full price of the missing or broken items. It is the student’s responsibility to thoroughly inspect the locker.

I must check out on or before the specified day with my IA. If I do not check out on time, I may be subject to a $50 fee and any charges associated with checking out of my locker.
