Please complete the form on the following page, print the completed form on your local printer, and attach required documentation for any courses you completed at another institution which you feel might fulfill Texas State's General Education curriculum requirements. Mail this information to Dr. Dave Nelson, University College, Texas State University--San Marcos, 601 University Drive, San Marcos, Texas 78666; fax it to Dr. Nelson at 512-245-8765; or bring it to the University College Office in the Academic Services Building, Room 100.

The documentation should include:

• a copy of the Texas State evaluated transcript which you received from the Office of Admissions. This should show both the original number of the course and what it was evaluated as.
• as much of the following as possible:
  • a copy of the courses's catalog description;
  • a copy of the Table of Contents pages from the textbook used in the course;
  • the course syllabi and/or course materials;
  • correspondence from the instructor explaining contents of the course.
