Mail this information to Dr. Dave Nelson, University College, Texas State University-San Marcos, 601 University Drive, San Marcos, Texas 78666; fax it to Dr. Dave Nelson at 512.245.8765; or hand deliver it to the University College Dean's Office in the Undergraduate Academic Center, Room 149.

The documentation should include:

• a copy of the Texas State evaluated transcript which you received from the Office of Admissions; this document shows both the original number of the course and the evaluated equivalent course
• as much of the following support as possible:

•  a copy of the courses's catalog description;
•  a copy of the Table of Contents pages from the textbook used in the course;
•  the course syllabi and/or course materials;
•  correspondence from the instructor explaining contents of the course.
