Appendix G: Assessment Form—Checklist for Faculty

Teaching Students Identified in the General

Education Portfolion Assessment Pilot Project

Spring 2004

To______________________________________:

The General Education Council and University College have initiated a portfolio assessment project.  The purpose is to provide an institutional assessment of the basic skills in Texas State's General Education Core Curriculum.  Dr. Donald Matlock, former chair of the Institutional Review Board, has determined this is an appropriate institutional assessment, which did not need the approval of the Institutional Research Board.

The student named below is enrolled in your class also listed below.  Please complete this form which will document which basic skills (reading, writing, oral communication, mathematics, critical thinking, ethics, and computer/information technology) are covered in your course.

If you do not incorporate a basic skill in your course, please place a √ in the column labeled "not applicable."  When you do employ a basic skill, please place √'s in the appropriate columns indicating how it is employed.

If you receive more than one copy of this form for students in the same course (i.e., Sociology 1310), please mark only one copy and staple all of the other copies to it.

Oral Communication
Mathematics
Computer/Information Technology
Critical Thinking
Ethics*

*A course has a component in ethics when it includes a study of the moral evaluation of human conduct, which addresses both the norms for evaluation and their application.

  • Select “Used in Grading” if your students receive a grade for this skill or must demonstrate the skill to earn their grades.
  • Select “In Syllabus” if you note this skill or expect its use by a specific reference.  For example, if you assign a textbook or other reading in your syllabus, check Reading in the “In Syllabus” column.
  • If you mention the importance of reading or refer students to written material, but don’t base any portion of their grades upon this skill, then select “Mention in Class.”

Thanks for your assistance in this important pilot project for general education assessment.

Sincerely,
