Exam Submission Guidelines

COMPLETING THE FACULTY EXAM REQUEST FORM

Before submitting exams to be scored, please complete a Faculty Exam Request Form and submit it with your exams and key sheet. Please note that TREC staff cannot make changes to a key sheet.

  • Review Option: Check the review item analysis to check your exam for possible mis-keyed (i.e., items incorrectly bubbled on the key sheet) or poorly constructed items. This choice is recommended because it allows the instructor to request that changes be made to the answer key or that items be dropped before the center scores the exams and generates output. If an instructor checks the no review option, the center automatically scores the exams and generates the output. The center will not rescore the exam should the instructor realize that some items have been incorrectly bubbled or poorly constructed.

  • Scoring Based on: Check the total number of points in the exam or check other and write in the total on the blank.

  • Roster: Check alphabetized if you want the roster in alphabetical order or student ID# if you want the roster ordered by Texas State ID; both options can be selected. The roster contains the number and percent of items each student answered correctly.

  • Score Distribution: Check one per section or one per form if you have multiple sections and/or forms being scanned and you want to examine the distribution of scores for each section or form separately.

  • Output: Check the format you would like for the output (electronic or hard copy); both options may be selected.

  • Response Sheets: Check correct answers if you want the response sheets to contain the correct answers or without correct answers if you do not want the response sheets to contain the correct answers for each item the student answered incorrectly. Response sheets are only available in electronic format.

  • Data File: Check the format in which you would like a data file for grading or record-keeping purposes of your exam scores. Leave this section blank if you do not want a data file. Data file for TRACS Post'Em is a new option. Choose scores only if you want to post students' exam scores. Choose scores and responses if you want to post both students' exam scores and response cards. To use this feature, students must bubble in a 6 digit student ID. If you are using old scan forms with room for nine digit IDs, ask students to right justify their IDs. In addition, you must include the Post'Em tool on your TRACS website.

  • Multiple Forms: Complete page 2 if you are using multiple forms that contain the same items in different orders and you do not want to bubble in the answers on multiple key sheets. Please note that you must provide one key sheet per form or section containing the necessary identifying information.

SUBMITTING SCAN FORMS FOR PROCESSING

Instructors must use the standard Texas State Scan Forms for their exams. You may purchase the forms from the Texas State University bookstore. Most departments purchase the scan forms in bulk and provide them to their faculty. Please note that we do not score exams administered using the Scantron Form 882 in our office.

To help insure the correct processing of test data, students must follow the following guidelines.

  1. Use only a No. 2 lead pencil when marking the Scan Forms. Do not use a ball point or a felt-tip pen.
  2. Make the marks dark and completely fill in the proper answer.
  3. Do not make any stray marks on the Scan Forms.
  4. Only one correct answer per question is allowed. If more than one response per question is marked, the scanner will read it as a blank answer and it will be scored as incorrect.
  5. Completely erase all mistakes. A poorly erased mistake may be interpreted as a response.
  6. The student's last name, first name, and middle initial must be entered in the blocks provided and the corresponding letters in the grid must be completely bubbled.
  7. Any other information that you want the students to write on the Scan Form may be entered in the blocks and grid provided for Course ID (course number), Sec. #, and Test Form.
  8. The student's Texas State identification number, up to six digits, should be entered in the blocks provided and the corresponding numbers in the grid must be bubbled. If you are using old scan forms with room for nine digit IDs, ask students to right justify their IDs.

Instructors must follow these guidelines.

  1. Do not reuse a key sheet if your test questions/answers have changed.
  2. When submitting scan forms for processing, enclose them in an envelope to protect them.
  3. All sheets, including the key sheet, must be oriented the same way.
  4. Do not fold, staple, or tear the Scan Forms.
  5. Place the completed Faculty Exam Request Form on the top of the exams.

PREPARING THE REQUIRED KEY SHEET

A key sheet (pink or purple Scan Form with the correct answers marked) must be prepared by the instructor and submitted with each group of tests to be scored. Key sheets are provided free and can be obtained at the TREC office. Some departments maintain a supply of key sheets and make them available to their faculty. A maximum of 150 questions may be scored for each exam. Please note that TREC cannot change or bubble in answers on the key sheet.

The following information must be on the key sheet for your exam scores to be scanned:

  1. Print your department or course name, course number (course ID), section number (Sect. #) and test form (if applicable) in the spaces provided. Bubble the corresponding letter or number in each grid. This information will appear in the report headings.
  2. Bubble in the correct answer for each question; only one correct answer per question is allowed. Please make sure that the answer sheet is bubbled completely. Questions not answered on the key sheet will not be scored.
  3. Provide one key sheet for each section or test form to be scored.
