Internship Forms

Directions: Attached are the required forms needed for the summer 2010 Internship. Please type or neatly print the required information. Use the attached “TECH 4390 Internship Checklist” as a guide for the April 1, May 1st and May 4th or 5th requirements.
April 1st Requirements – Qualifying for the Internship
Turn all requirement materials in to Deb Howell, who is located in room 2240 of the RF Mitte Building. Also, be sure to apply for your official “Degree Audit Report” from the College of Science and Engineering about one month in advance. This can be done at http://www.science.txstate.edu/advising. Select “Forms” on the drop down menu and click on “Generate Degree Audit Report,” then “Reason”: TECH 4390 Internship and “Must Select”: I would like to pick up my signed DAR (202 Centennial Hall).
May 1st Requirements – Documenting Internship Company
Submit:
Appendix B if you have found an internship
Appendix C in the rare instance where you have not yet found an internship by the May 1st deadline, but can list at least six (6) companies you have contacted with the names of the contact persons
May 4th or 5th – Requirements for the Summer Internship
Sign up for a one-hour meeting where each requirement for the Internship is discussed in detail. This will include:
Goals and Objectives
Company Organization Chart
Daily Work Logs
Mid-Term Evaluation
Course Summary Paper
Final Evaluations
Grading Criteria
May 4th Meetings:

Sign-up sheets for these meetings are available from Deb Howell. Most students will sign up for one of these meetings when they submit their April 1st “Required Internship Materials.”
