Nomination Form for Excellence in Diversity and Inclusion Award

1. Give a brief professional sketch of the nominee and of his or her present position:

(Please do not submit a full vita or resume.)

 

2. Discuss a, and b below with regard to the nominee’s:

 

a. critical support that assists diversity and inclusion efforts with faculty, staff, or students

 

(Note: if the nominee’s primary job duties are related to diversity, the selection committee will consider only those activities beyond the normal scope of their duties. Examples of activities of past award recipients include new or innovative programs of inclusion or diversity, community service activities, and new or creative curriculum approaches.)

 

b. sustained commitment to diversity and inclusion through research, programs, services, activities, and new initiatives

 

(Note: if the nominee’s primary job duties are related to diversity, the selection committee will consider only those activities beyond the normal scope of their duties. Examples of activities of past award recipients include a history of community involvement in activities related to diversity and inclusion and a pattern of service that champions Texas State’s commitments to diversity and inclusion.)

 

 

3. Provide no more than three letters of support from colleagues, co-workers, and students. (Required for all nominations, including self-nominations.)

 

 

Nomination packets are due by 5 p.m. on March 30, 2018. Deliver to the Office of Equity and Access, JCK 164 or send by e-mail as a single PDF attachment to Dr. Luizzi, vluizzi@txstate.edu.

EXCELLENCE IN DIVERSITY AND INCLUSION AWARDS

NOMINATION, SELECTION AND AWARD TIMELINE

 

 

DATE ACTIVITY

 

February 15 Chair of the selection committee (Chair) notifies academic and staff administrators of the award and encourages them to submit nominations.

 

Chair sends similar notice to Equity and Access Committee

 

March 30 Chair completes review of nominations for completion and makes nominations available to selection committee.

 

April 21 The selection committee will review all nominations and submit its evaluation of each candidate to the Equity and Access Committee. The Equity and Access Committee will determine its first, second, and third choices for the staff recipient and likewise for the faculty recipient.

 

June 1 The Chair will submit the choices to the President via the Provost.

 

August 15 The president will notify the award recipients and announce the results to the university community.
