Student Application

The "Rising Stars Algebra Program" is an exciting opportunity for students to learn algebra in the 6th grade using the Mathworks *Math Explorations* curriculum. Students will learn algebra early, preparing them for more advanced maathematics and higher-level problem solving.

Students will need to be entering the 6th grade during the academic school year of 2011-2012 and attend Miller Junior High School in San Marcos, Texas. The main qualification is a willingness to work hard and persevere when faced with challenges. All interested students are invited to apply.

Parent / Student Agreement

Parent: I give permisison for my child to attend the Mathworks Rising Star Algebra program. I understand that he(she) will need to turn in all assignments, and may need to do extra work to participate in this accelerated program.

Student: I will complete the assignments and actively participate in the Mathworks Rising Star Algebra class. I will alert my parents and my teacher if I find that I need additional help to understand the assignments.

Submission of this application is proof that you agree to the terms.
