Spring 2013 Application


APPLICATION FOR ACADEMIC COURSE CREDIT AT THE ROUND ROCK AND SAN MARCOS CPM LOCATIONS – REQUIRED FOR SPRING 2013 SEMESTER COURSES

As a part of the implementation of CPM courses into the Texas State University “Banner” Registration System all students wishing to receive academic credit for a CPM course offered in Round Rock or San Marcos must complete an online application form and file it electronically with Texas State University’s Office of Distance and Extended Education. 

Go to http://www.extension.txstate.edu/CPM-application.html to complete this form.  Please contact Debi Perkins dp01@txstate.edu (512/245-2507) from the Office of Distance & Extended Education if you have any problems with the completion of this form.  Please copy Dr. Howard Balanoff, Director of the CPM Program at hb02@txstate.edu (512/245-3453) on any correspondence that you have with Distance and Extended Learning.

Completion of the online form will allow you to receive a registration code which permits you to register for CPM courses in Round Rock and San Marcos for Spring 2013 in the Banner System. 

Please go the schedules link for a list of CPM courses in Round Rock and San Marcos that will be offered in the Spring 2013 semester.  A list of courses that will be offered after the Spring 2013 semester is also provided.

Registration for all courses for the Spring 2013 semester (including CPM courses) opens with the regular Texas State University registration period starting on October 11 and continues to November 9th.  After that time late registration periods will be in effect.

Once the CPM Application form is completed you will be able to go to the “Banner” Registration System and register for CPM courses in San Marcos and Round Rock.  The cost for undergraduate CPM courses is $631 and the cost for graduate courses is $801.  An additional $25 is charged by the Office of Distance and Extended Learning for the completion and submission of the CPM Application.
