Instructions

Select the appropriate course and fill in the requested information. Both courses are available in print-based and online formats.

Individuals who enroll in the online version will be sent an e-mail with a password and user ID. Note: The on-line course cannot be accessed without the password and user ID.

Individuals who enroll in a correspondence course will be mailed a workbook that contains the exam.

Participants who do not complete the coursework within 90 days will need to re-enroll.

If your license number is from a neighboring state and only contains 5 digits, add a "0" [zero] at the beginning to make 6 digits.  You will be required to enter all 6 digits when submitting forms to THMP.

If you are an administrator or other professional without a license number, enter "000000" (six zeros) for the license number and "Not Applicable" for the issuing state.

Information Release

As an applicant you understand your completion of this training will be reported to the Texas Medicaid Healthcare Partnership (TMHP).  Health and Human Services Commission (HHSC) representatives request completion status of certain personnel as a part of their regulatory enforcement.  Your submission of this registration form indicates your understanding that Texas State University will release completion information to authorized HHSC personnel.
